RCGP Secure Environments Group 8th Health & Justice Summit: Communities of Practice in Secure Care

To address the complex health and social care needs of people resident in custodial settings, effective partnership work is required not only between health partners and the prison service but also with health and justice policymakers. PHE 2018-2019 Annual Health & Justice Review

Prisons and other secure settings can provide a unique opportunity to address a wide variety of health needs. These can range from infectious diseases such as HIV, hepatitis and tuberculosis as well as mental health issues, substance misuse disorders and long-term conditions.

‘Prison Health’ should be considered ‘Public Health’ as the vast majority of prisoners return to the community, and the investment in healthcare in these settings provides a wider overall benefit. The mission of this Summit is to share models of good practice, innovations in healthcare delivery and advances in treatment in order to best utilise the opportunity we have for our patients and for our society.

Following on from the highly successful 7th RCGP Secure environments summit in November 2019, and the 1st Virtual RCGP Health and Justice Summit in November 2020 the RCGP Secure Environments Group & Convenzis are very excited to share details of the 8th Annual RCGP Secure Environments Summit taking place on Tuesday the 12th and Wednesday 13th of October 2021 in central Newcastle.

This as always is run with various key partners, this includes Ethypharm, Mountain Healthcare, Spectrum Healthcare, Nottinghamshire Healthcare, Ashworth Hospital, Mersey Care, Merck Sharp & Dohme, Inclusion (Part of Midlands Partnership NHS Foundation Trust), Oxford Immunotec, SLD Training, Essex Partnerships, Public Health Wales and West London NHS Trust.

In recognition of the significant progress and collective contribution of our attendees, partners and sponsors, we are titling the 8th Summit:

We are now considering the next the next decade, and we look forward to receiving content for the 8th RCGP Secure Environments Group conference on the following themes including:

Plenary Session Alison Lloyd & Lee Partis Being OUT Spoken Addressing trauma in prisoners through the delivery of a specialist trauma informed talking therapy service in the HMP estate. Survivors Manchester works to meet the therapeutic and advocacy needs of boys and men affected by sexual abuse, rape and sexual exploitation, including those in prison. Using its knowledge and experience, the organisation has widened its offer in prison to create a new partnership working process.

Plenary Session Rachel Moore, Sammy Wilson and Steven Knight Accelerator Prisons Project The Accelerator prisons project is delivered by the Reducing Reoffending, Partnerships and Accommodation Directorate in HMPPS. Funding has been awarded to design, implement and test new ways of improving the support provided to people in prison, helping them to desist from crime on release and to rebuild their lives. The project provides an opportunity to work collaboratively with partners and stakeholders, strengthening relationships and innovate. This presentation will offer the opportunity to find out how the Health and Substance misuse theme is progressing so far.

Plenary Session Dr Sarah Bromley, Medical Director Health in Justice Practice Plus Group Ralph Lubkowski – Governor at HMP Hewell Samantha Hewitt – NHSE West Midlands Anita Thomas – MPFT A New Model for In-patients Following the closure of an in-patient unit in a busy remand prison due to IPC concerns, provider, commissioner and prison operator worked in partnership to design a new model to care for vulnerable and unwell patients. This presentation explores the model, its evaluation and proposes a new way of working.

Stream A – 35 Minutes Workshop 1: Pamela Walters, Clinical Director at Forward Trust Buvidal pilot This is an overview of setting up a prolonged release buprenorphine pilot, (Buvidal in this case) within a busy local remand prison. Workshop will include the challenges pulling together the business case, negotiating pathways and the learnings from the pilot so far with some information on initial outcome measures for service users. Workshop 2: Caryl Watkins – Drug Strategy Manager at G4S or Mike Vigar – Head of Rehabilitation at G4S Whole system approach to supporting PS users in custody Sharing practice in safeguarding and supporting users of PS in custody and understanding reasons for use Workshop 3: Dr Nat Wright, Clinical Research Director – Spectrum Community Health CIC Coroners Court Training – A Taster Session Spectrum’s training arm, SLD Training, have designed Coroner’s Court Training to provide the core knowledge and skills required to enable individuals to be confident and capable witnesses at a Coroner’s inquest. In this insightful taster session, you will learn more about the course, and how we can support your organisation at this challenging time.

Stream B – 35 Minutes Workshop 1: Professor Tammi Walker, Principal of St Cuthbert’s Society and Professor of Forensic Psychology, Durham University and Professor Carolyn Chew-Graham, Professor of General Practice Research, School of Medicine, Keele University Physical Health Inequalities in Imprisoned Women with Serious Mental Illness: How can prison and community general practitioners work together? Imprisoned women have multiple complex health and social care needs; however, no reliable data exists reporting whether women with severe mental illness in prison access primary care services concerning their physical health care needs and whether these needs are adequately met. This paper will propose how prison and community GPs could work together. Workshop 2: Dr. Marcus Bicknell KEEPING CHILDREN SAFE IN SECURE CARE’ This workshop describes the circumstances in which young people enter secure care and how they are managed. It will focus upon how they are kept safe, the specialist care which they receive and how specific risks are reduced. Physical health, mental health, emotional wellbeing and sexual health will be discussed. Anonymised case studies will be explored. Innovations in care delivery, harm prevention and reduction and successful team working will be discussed. Barriers to care will be considered. It is intended that this workshop will contribute to children’s safeguarding CPD for delegates (a certificate will be provided). Dr. Bicknell cared for young people in secure care in Nottinghamshire for 6 months in 20202 and 2021 during the second wave of Covid infection. This workshop will consider some of the additional issues which these unique circumstances created. Workshop 3: Mark Gillyon-Powell, Head of Programme – HCV Elimination at NHS England and NHS Improvement Hepatitis C testing – how to make it a success in your prison. HMP Bronzefield case study. The team involved will describe their roles and responsibilities in their successful hepatitis C testing programme. A best practice sharing session with input from the hospital clinical team, the prison healthcare team, The Hepatitis C Trust, and the Head of Programme HCV Elimination (Mark Gillyon-Powell).

Stream C – 35 Minutes Workshop 1: Carolyn Houghton, Head of Prisons and Criminal Justice at Rethink Mental Illness & Jessica Redhead, Senior Commissioning Manager, NHS England & Improvement Implementing Reconnect in the North East during Covid19 – Sharing learning, reflections and best practice Sharing the learning, reflections and best practice from North East as they mobilised their Reconnect national pathfinder during Covid-19. Highlighting the benefits of hybrid working in the prison and through the gate to support prison leavers for 6 months post release, working in partnership with HMPPS, PCCs and LAs. Workshop 2: Dr Shamir Patel, Divisional Clinical Director: Specialist Services – Barnet, Enfield & Haringey Mental Health Trust (inc. North London Forensic Service) and Drs Bartlett, Chao & Hunt-Grubbe (Consultant Forensic Psychiatrists) Cognitive Impairment in Secure Settings – Pathways of Care The population of the UK is ageing and this is reflected by populations in secure settings. Commissioning is different for prisons, hospitals and the community, which complicates the management of this group. This workshop plans to look at current practice, which is fragmented and to consider ideas for change. Workshop 3: Dr Nat Wright, Clinical Research Director, Transform Research Alliance, Visiting Associate Professor, Leeds University, Non Executive Director, SLD Training & Dr Craig Sayers, Clinical Lead Prison Healthcare at NHS Forth Valley (Confirmed) Challenges to Initiating Prolonged-Release Buprenorphine in a Custodial Setting

Stream D – 30 Minutes Workshop 1: Bethan Leach, Dietitian, Health in Justice Thames Valley Region, Practice Plus Group and Stephanie Cogbill, Associate Nutritionist registered with the Association for Nutrition (AfN). Nutritional provision and food choice in 2 adult male UK Prisons There are well-documented links between poor nutrition and chronic disease, which is a feature of prisons. This workshop collates nutritional analysis work in 2 male prisons and examines how the food provision, and food choice, compares to the UK recommended dietary reference values (DRV’s) Workshop 2: Cheryl Grote – Clinical Practice and Teamwork: The Physical Health Hub Workshop 3: John Wakelam, Pharmacy Services Manager at West London NHS Trust and Katie-Lynn Harfield, Medicines Management Nurse at West London NHS Trust Safety in Medicines Management An overview of the improvements in key aspects of medicines safety within a High Secure Hospital. Welcoming a nurse into the Pharmacy team enabled a collective theoretical and practical approach to medicines management; proving effective not only in meeting requirements, but raising the quality of practice far beyond expectation.

Stream E – 35 Minutes Workshop 1: Dr Karen Massey, Named Doctor for Safeguarding, Spectrum Community Health, HMP Preston Dr Joy Shacklock, RCGP Safeguarding Representative, Named GP for Safeguarding Harrogate and Rural District, Homeless Health Lead, Harrogate Dr Peter Billingsley, Prison Doctor HMP Full Sutton and Named GP for Safeguarding Scarborough and Ryedale A practical look at safeguarding adults in the secure estate This interactive workshop will involve case-based discussion on common safeguarding scenarios in the secure estate. We will explore different types of abuse and how they might be identified and managed. The discussions will enable examples of good practice to be shared by the participants and identify areas for change. Workshop 2: Dr Russell Green, Medical Director Mental health and Substance Misuse at Practice Plus Group Understanding and Working with People with Personality Disorder This workshop will aim to provide a practical understanding of the psychological processes involved in personality disorder. It will explore how these impact on the relationships between our patients and healthcare services and how understanding these can improve the therapeutic relationship and reduce conflict. Workshop 3: Dr Michael Frost MD, FACP, FASAM, President and Medical Director of The Frost Medical Group ‘Steps to the implantation and removal of Sixmo (buprenorphine) Implant This workshop will introduce the use of implantable buprenorphine for the treatment of opioid dependence and will familiarize the attendees with the procedure for insertion and removal of the buprenorphine implants.

Plenary Session Dr Cat O’Callaghan, Consultant Clinical Psychologist, Inclusion, Midlands Partnership Foundation Trust and Mark Smith, Acting Clinical Lead, Inclusion, Midlands Partnership Foundation Trust Delivering the “Secure Stairs” project at HMYOI Werrington “Secure Stairs” is a framework that facilitates a trauma-informed way of working with the young people at HMYOI Werrington. Baseline training in attachment theory and developmental trauma, good multi-disciplinary/multi-agency working, and the opportunity to undertake reflective practice for all staff, helps us to develop a greater understanding of how to best meet the needs of those in our care.
