Just like in every big organization, how to get the most out of data is a key question for NHS leaders. We have been working with cities through the Bloomberg Philanthropies City Data Alliance program for the past year and we believe there are key takeaways from that experience which can apply to the NHS context.
Through this programme we helped 3 cities mature and scale their data practices. We coached them in the process of building a beacon data service, a data service standard and toolkit, and supported them in their journey to create a data community of practice. In this presentation we will explain how each of these outputs helped the cities evolve their strategic use of data and how some of these lessons can help NHS leaders shape key questions about their own data practices.

These principles underpinned a national programme of work undertaken by NHS Arden & GEM CSU and the Department of Health and Social Care to enable local authorities to submit standardised adult social care data into a secure environment to be linked with health data.
In this session we will cover some of the challenges this programme presented, how they were overcome and how organisations are being supported to submit and access data on an ongoing basis, providing a bridge between health and social care
We will also hear from Liverpool City Council on the practical impact of the programme and how the data is being used for operational oversight, national monitoring, commissioning and research.

The Impact of the High Risk Adult Cohort (Confirmed)
Using joined up data to understand how the system works and to identify a cohort of the population which we have named “High Risk Adults” who are high intensity users of the service and who have some easily identifiable “triggers” so that our Clinical Leaders can design and implement Community based services which reduce the number of admissions and therefore Hospital Bed Occupancy.
In the talk I will identify the cohort of patients, identify the triggers, as well as the services that have been put in place across the Wakefield and District population and crucially I will be able to demonstrate the impact that our work has had.
