Charlotte Howard

Charlotte is a Physiotherapist by background who currently works as Hospital Discharge Lead for Blackpool Teaching Hospitals. She has over 10 years experience in rehabilitation, patient flow and discharge planning with an interest in service improvement and re-design, keeping the patient at the heart of everything she does. She is passionate about creating and leading services delivering integrated, collaborative and compassionate care providing a seamless transition for patients on their Hospital to Home journey. She truly believes and advocates for co-ordinated Health and Social Care, working together with colleagues towards developing joint pathways providing quality discharge experiences for patients on the Fylde Coast.
