Kylie Farbrace

Kylie Farbrace is Director of Operations for Planned Care at Lincolnshire Community and Hospitals Group. A physiotherapist by background, she brings over 25 years’ experience spanning frontline clinical practice, community rehabilitation, professional leadership and operational management.

Her early career in community physiotherapy, intermediate care and falls prevention shaped her commitment to helping people maintain their independence. She went on to lead AHP services at Care Plus Group and became Assistant Clinical and Operations Director at Provide CIC, overseeing a diverse portfolio of adult and children’s community services and leading transformation across organisational boundaries. Before taking up her current role, she served as Chief Allied Health Professional at Lincolnshire Community Health Services. Her work has included developing award-winning rehabilitation services, with recognition for service redesign and patient involvement.

Alongside her NHS career, Kylie worked as a consultant physiotherapist with the Football Association, supporting England women’s international youth teams at World Cups and European championships.

Kylie is now supporting integrated neighbourhood working in Lincolnshire through district nursing transformation, closer working with primary care and the development of community rehabilitation pathways. She is passionate about multidisciplinary working, developing the nursing and AHP workforce, and turning neighbourhood ambitions into practical improvements that deliver personalised care closer to home.
