Nicola Luhar is an Advanced Clinical Practitioner and Joint Clinical Lead for the Admission Avoidance Team (AAT). Having worked within the service for a number of years, she brings significant experience in delivering community-based care, service development, and multidisciplinary working. Nicola is passionate about preventing avoidable hospital admissions and improving patient outcomes through safe, timely, and person-centred care.
