Sarah McCulloch (she/her) is Pathway Lead for the Barnet 18-25 Transitions Pathway at North London NHS Foundation Trust. An occupational therapist by background, she leads service development focused on improving continuity of care for young people moving between child and adolescent and adult mental health services. Her work centres on practical pathway design, relational support, and reducing disengagement at points of transition. She is also co-author of an accepted article for Mental Health Practice on the development of the Barnet 18-25 Transition Pathway.
