Following the Covid outbreak, the NHS Long Term Plan ambition to avoid unnecessary face to face outpatient attendances has become an urgent imperative, these targets include:

  • Removing the need for up to a third of hospital outpatient attendances a year by 2023/24
  • Avoiding up to £1.1bn of additional expenditure.
  • And ensuring all patients can access digital outpatient care.

Find out how Swedish Healthcare provider Bra Liv Nära is positively transforming patient pathways using digital tools and new ways of working.

Bra Liv Nära were the first publicly financed digital healthcare provider in Sweden (and Europe). They started their journey back in 2015 and today cover a population of 365 000 patients of which 42 % have downloaded the Bra Liv Nära patient app. In this session Ulf Österstad will share Bra Liv Nära’s story and how they have succeeded in improving demand management and outcomes for departments and healthcare teams whilst giving patients easier and better access to care. Ulf Österstad and Karen Jones, regional business manager of Visiba Care, the virtual care platform used by Bra Liv Nära, will talk about how these learnings can easily be applied to other parts of the healthcare system.

My Medical Record – 8 years experience of PIFU. What works, what doesn’t?

UHS has run a virtual patient service for around 8 years for a range of clinical pathways. This has resulted in a better experience for patients and clinicians and the removal of many thousands of unnecessary appointments. The service has around 140,000 users.

Transforming Real Estate and Patient Care

Have we ever lived in a world where there is so much in the way of disruption – new technologies, conflict and political upheaval, of course the massive global impact of Covid on our lives, not only financially but in so many other ways, on the NHS, on the way we live, learn and work, disorder within global supply chains and transportation. Within healthcare virtual consultations are rapidly gaining popularity, in part because the arrival of COVID-19 has highlighted the value of remote appointments, but also because the benefits of video consultations are finally being recognised. Backed by a strong policy push from the government to increase the use of digital technology, healthcare services are finally in a position to keep pace with the modern world with significant improvements in both efficiency and patient care.

Back to the Future – a journey into the pandemic and beyond

In 2017, an overwhelming 95% of patients in West Yorkshire said they wanted an alternative to face to face care where appropriate. In response, CHFT and its partners launched an ambitious programme to radically change the way in which outpatient care is delivered by maximising digital opportunities to create a service that fits with people’s busy and often complex lives. By working with service users to co-produce pathways, the Trust now delivers circa 50% of outpatient attendances through a remote or virtual option, making services more accessible to users when they need them the most. The session will provide an insight into the Trusts journey before the pandemic, and how the learning and models provided a platform for covid recovery and achievement of the 22/23 Planning Requirements.

Virtual Outpatients; clearing the Covid backlog

One of the many consequences of Covid is a record number of patients awaiting outpatient appointments. Digital triage of these appointments enables specialists to triage the needs of their patients, to pre-order investigations, and remotely manage patients where appropriate, moving knowledge not people, improving the efficiency of the system and saving unnecessary trips to the hospital.
