Improving patient outcomes through Discharge to Assess (D2A) and Continuing Healthcare (CHC) process

An opportunity to network with colleagues and understand the key challenges faced today in managing patient flow and how we, as a Commissioning Support Unit (CSU) or Integrated Care Board (ICB), can work in partnership to support this.

My session will be looking at how ICB delivered Discharge to Assess pathway can support the patient transition from an acute setting into a community environment, whether that is in the person’s own home or care setting. I will outline the process the CSU undertakes on behalf of ICBs to facilitate this including supporting the person’s ongoing recovery and completion of assessments to determine their long-term care needs.

We all use the NHS and may at some point may be an inpatient. As professionals we want the best outcomes possible for patients and families at what can be very stressful periods in a person’s life.
