Current Landscape:
With the expansion of virtual wards, NHS hospitals have successfully avoided a significant number of hospital admissions. Virtual wards allow patients to receive treatment from the comfort of their own homes, thus freeing up hospital beds and alleviating pressure on NHS resources. In 2025, this model of care has become even more important due to the growing demand for NHS services from an ageing population and the limited availability of hospital capacity. Virtual wards offer a proactive approach that not only addresses immediate patient needs but also contributes to the NHS's broader goals of improving patient outcomes, reducing hospital congestion, and enhancing care accessibility.
Importance and Timeliness of the Event:
The Virtual Wards Conference is critically timed to bring together senior healthcare professionals, policy leaders, and industry stakeholders to address the most pressing challenges and advancements in the sector. As the NHS continues to promote virtual wards as a sustainable healthcare solution, this conference will serve as a key forum for collaboration, knowledge-sharing, and strategy development among NHS peers. In 2025, virtual wards are no longer just an innovation but a necessary element of healthcare delivery. This event offers a vital opportunity to refine and improve virtual ward strategies, enabling NHS leaders to drive meaningful change in patient care and service delivery.
This event serves as a timely platform to promote innovation and collaboration, which is crucial for the effective management of virtual wards within the NHS and the overall improvement of the healthcare industry.

Case Study - Hospital at Home in action: A case study of patient referral, onboarding, and follow-up for asthma and cardiac care (Confirmed)
Case Study - Luscii HealthTech
Discover how South Tyneside and Sunderland NHS Foundation Trust, in partnership with Luscii, is reshaping at-home care for asthma and cardiac patients. We’ll show how Luscii’s app works with the KardiaMobile ECG device to bring real-time monitoring into patients’ homes, reducing hospital visits and empowering self-management. You’ll hear about our co-design process with Same Day Emergency Care teams, GPs, and patients, and learn how the Atrial Fibrillation pathway was tailor made to meet individual needs.
Key takeaways:
· Hospital at Home in action: A case study of patient referral, onboarding, and follow-up for asthma and cardiac care
· Luscii + KardiaMobile ECG: How real-time data flows from patients’ homes to clinicians, enabling early intervention.
· Co-design benefits: Examples of how collaboration improved Same Day Emergency Care, GP workflows, and patient experience.
· Measurable impact: Reduced A&E attendances, better asthma compliance, and faster cardiac responses through digital monitoring.
Keynote Presentation - SMART Hospital @Home/Virtual Ward Concept: Generalised Approach (Confirmed)
The idea of virtual ward as it operates in Medway NHS Foundation Trust (MFT) is a concept that breed the use of technology to offer hospital treatment and care to patient, at the comfort of their own homes.
It is a hospital level care that make us of electronic monitoring device to offer remote monitoring, while offering best of care. SMART virtual ward is unique in MFT because it is multidisciplinary, involving all the medical and surgical teams working together to offer acute response, recovery and rehabilitation to patient who are ordinarily should have been admitted in hospital bed; hence the name SMART (surgical medical acute recovery/rehabilitation/recovery team).
This has successfully kept patients out of hospital, reduce front door pressure in A and E and acute areas, decongest in-patient beds, free up hospital bed for more acutely unwell patient and ensure hospital avoidance- especially with our community virtual ward team in Medway and Swale. This model has also enabled the team to established various patient centred pathways, that allow acutely unwell patient to receive hospital level treatment and care without a necessary need for hospital admission, while using safety net as a key for escalation in event of deterioration.
