Population Health is an approach that aims to improve physical and mental health outcomes, promote wellbeing, and reduce health inequalities across an entire population.

A relatively new approach called Population Health Management (PHM) is helping the NHS understand our current, and predict, our future health and care needs so actions can be made to tailor better care and support to individuals, design more joined-up and sustainable health and care services and make better use of public resources.

PHM is a partnership approach across the NHS and other public services including councils, the public, schools, fire service, voluntary sector, housing associations, social services, and police. All have a role to play in addressing the interdependent issues that affect people’s health and wellbeing.

For example, adults and children who live in cold, damp housing may be more likely to develop respiratory problems over the next 20 years because their lungs are affected by the mould spores in their homes. If we improved their housing now, by working with partners such as local councils and housing associations, they may not end up with various health conditions in the future which can result in poor quality of life (conditions like asthma, chest infections, and other respiratory problems) and could avoid the need for multiple health and care services.

Implementing population health management services is a key focus within the NHS long term plan and is supporting ICSs’ to understand the areas of greatest health need and match NHS services to meet them. These solutions are becoming increasingly sophisticated in identifying groups of people who are at risk of adverse health outcomes and predicting which individuals are most likely to benefit from different health and care interventions, as well as highlighting health inequalities.

Our upcoming conference aims to provide updates and clarity on this developing strategy and share best practices and outcomes from across the NHS Ecosystem.

The Art of the Possible – My Personal Population Health Management Journey (Confirmed)

After spending seven years supporting systems across England build capacity and capability, I left NHS England to return to Wales and do population health management for ‘real’. The health boards in Wales are truly integrated and public health is now incorporated into the Health Board.

We have an opportunity, as Marmot Region, to use population health management approach to really target rising risk groups and make a difference to the lives of the people of Gwent. We’ve commissioned a small test and learn to explore the art of the possible. We have combined long-term conditions with weight management and exercise to understand how to fully support people.

During this session Chris Elkin will provide an overview of the Piota Healthcare App platform and how it can be used to help protect and connect your citizens with their communities, your Healthcare partners and service providers.

Using data to identify barriers, motivate change and adapt digital interventions in response to evolving behaviours.

This presentation will look at how data collected via an app can be used to identify those who may be most at risk of long term health conditions due to physical inactivity, weight management issues and addictive behaviours. Illustrated with case studies from UK local authorities, this presentation will show how people from more deprived communities can be engaged via an incentivised digital behaviour change intervention.

How people can be encouraged to share information through app-based surveys and record their activities over time. Creating a rich data set which can then be used to tailor incentive programmes to support those most in need and adapt as their behaviour changes.
