The Convenzis Integrating Health and Social Care Conference is a showcase of best practices from teams across the NHS and provides a platform to meet and engage with sector-leading guest speakers, forward-thinking innovators and 200+ peers from across the NHS.

We are living through the greatest challenge our health and care system has ever faced, yet, even in crisis conditions, everyone working in our health and care system has continued to deliver excellence. Critically, collaboration across health and social care has accelerated at a pace showing what we can do when we work together, flexibly, adopt new technology focused on the needs of the patient, and set aside bureaucratic rules. Integrated care is about giving people the support they need, joining up across local councils, the NHS, and other partners. It removes traditional divisions between hospitals and family doctors, between physical and mental health, and between NHS and council services.

Transforming the delivery of health and social care is needed to address the growing pressures due to funding constraints and the increasing complexity of needs for services. In an attempt to address these issues, various programs to integrate health services have been developed internationally, including, The World Health Organisation Framework on integrated people-centred health service and the NHS Long-Term Plan.

Since April 2021, all parts of England have been covered by one of 42 ICSs, some systems have already been working as ICSs for a number of years, while others have evolved from STPs more recently. In all cases, the ways of working and governance structures underpinning ICSs remain a work in progress.

Join us at the Integrated Care Summit as we continue the debate on the latest government plans to integrate health and social care providers to help improve healthcare for all.

The 2023 Summit provides a timely platform for service providers to meet up alongside NHS stakeholders to help drive improved collaboration and interoperability across the UK health system.

The recent formation of the 42 Integrated Care Systems, ushers in a new dawn of opportunity and innovation for the NHS. With a blank technological canvas ready to be painted on, now is the time to stand tall and run forward, embracing new ideas and delivering leading edge capabilities from large technological progresses.

Whether its Patient Flow, Population Health, Procurement as Scale, or Patient Outcomes there are so many new ways of delivering improvements. The one thing we shouldn’t get hung up on, is how we dealt with these things in the past.

Presentation - Challenges facing Secondary Care Pharmacy within the ICS (Confirmed)

Presentation to highlight the challenges faced by secondary care Pharmacy services in delivering the ICS purposes.

Specifically related to the Humber and North Yorkshire ICS, which is one of the largest by geographical size, but has a small population, the challenges are in providing a digital platform, accessible by all, to enable the efficient use of virtual clinics/consultations and requirements for medicines to be supplied to patients across large distances.

The presentation will look at the reasons, having a population base scattered over rural areas serviced by multiple hospital sites has on staff resources and duplication of work, and will also delve into current and future options to help address health inequalities due to access and demographics, funding issues, and NHS net zero.

Open, networked, and dynamic innovation paradigm for an integrated public health system (Case of Nutrition) - (Confirmed)

In the last decades, globalization deepened the role of technology and innovation in addressing societal, health and global challenges.

For most of the 20th century, the old “closed innovation paradigm” was adapted by health systems, this approach worked and worked very well. But today, embracing an open innovation paradigm” is crucial to drive systemic and dynamic transformation into health and social care systems towards success and sustainability.

An example from public health nutrition highlights how this open approach leads to many potential opportunities for the public health system but also to new challenges for people,practitioners, decision- and policy- makers.

Finally, this talk will show a modern comprehensive scheme based on open innovation approach to rethink public health systems (case of nutrition) with trends, concepts and methods coming from different disciplines including digital media, health communication, science journalism, artificial intelligence, and behavioral economics among others. Strengths, limitations, and drawbacks of the approach are discussed.
