NHS Population Health Management Conference
The Convenzis NHS Population Health conference provides a timely platform for NHS professionals from across the sector to network and discusses the PHM strategy and how it is helping to identify healthcare inequalities and how we can overcome this area of concern as a collective.
Population Health is an approach that aims to improve physical and mental health outcomes, promote wellbeing, and reduce health inequalities across an entire population.
A relatively new approach called Population Health Management (PHM) is helping the NHS understand our current, and predict, our future health and care needs so actions can be made to tailor better care and support to individuals, design more joined-up and sustainable health and care services and make better use of public resources.
PHM is a partnership approach across the NHS and other public services including councils, the public, schools, fire service, voluntary sector, housing associations, social services, and police. All have a role to play in addressing the interdependent issues that affect people’s health and wellbeing.
Implementing population health management services is a key focus within the NHS long term plan and is supporting ICSs’ to understand the areas of greatest health need and match NHS services to meet them. These solutions are becoming increasingly sophisticated in identifying groups of people who are at risk of adverse health outcomes and predicting which individuals are most likely to benefit from different health and care interventions, as well as highlighting health inequalities.

Population Health Management and Impact Ability Modelling
In this session, David and Jane will explore some of the challenges that integrated care systems are facing and how the role of impact ability modelling can support improvements in the health of a population. Impact ability modelling is critical to support the design and implementation of strong and tailored interventions that aim to have maximum impact on a targeted population.
Impact ability modelling is at the core of the NHS Population Health Management programme, endeavouring to use data-driven insights and evidence of best practices to improve the health and wellbeing of our populations
Nothing about us, without us, is for us.

Delivering National and Regional Patient Engagement
iPLATO’s myGP Population Health Service is a market leading multi-media mass communication and online booking solution used by NHSE/I, and ICBs to engage with the population about key health services. It has been recommended for use nationally in 'The Independent Review of Cancer Screening Services' by Sir Mike Richards.
It has proven to increase the uptake of national screening programmes, health checks, vaccination roll-outs and has played a key role for much of England for mass communication during the COVID-19 outbreak.
This session will look at two case studies utilising technology solutions for Cancer screening and Diabetes prevention.

Delivering the aims of the NHS Long Term Plan
The NHS Plan is still at the heart of the change we need to see in order to give the NHS longevity. How effectively is the Plan being embedded in delivery in the new structure and are we as yet seeing the results of new ways of working. If not how could some of the critical principles be fast tracked and how do we begin to measure the impact of the change.
