Tirwedd Gyfredol a’r Heriau:

Mae GIG Cymru ar foment dyngedfennol yn ei thaith trawsnewid ddigidol. Er bod yr uchelgais ar gyfer system iechyd a gofal gysylltiedig, wedi’i gyrru gan ddata, yn glir, mae’r llwybr i’w chyflawni yn dal i ddatblygu. Mae’r rhan fwyaf o fyrddau iechyd Cymru yn dal i ddibynnu ar systemau papur neu offer digidol arwahanol, gan greu rhwystrau i ryngweithredu, rhannu data, ac effeithlonrwydd clinigol.

Yn wahanol i rannau eraill o’r DU, lle mae rhaglenni rhanbarthol a chenedlaethol eisoes ar waith, mae’n rhaid i Gymru nawr wynebu penderfyniad strategol hanfodol: a ddylai’r wlad fabwysiadu un datrysiad unedig o ran Cofnod Claf Electronig (CCE), neu rymuso mentrau rhanbarthol a lleol wedi’u teilwra i anghenion a chyd-destunau penodol?

Mae cymhlethdod strwythur GIG Cymru – gyda byrddau iechyd unigol sy’n meddu ar lefelau amrywio o aeddfedrwydd digidol – yn gwneud y cwestiwn hwn yn fwy brys ac yn fwy cynnil. Gallai’r dull anghywir arwain at fuddsoddiad wedi’i wastraffu, gweithredu hirfaith, ac effaith fach ar ganlyniadau i gleifion. Gallai’r dull cywir agor y drws i welliannau trawsnewidiol mewn cydlynu gofal, diogelwch cleifion, ac effeithlonrwydd y system iechyd.

Amseroldeb y Digwyddiad:

Mae’r uwchgynhadledd hon yn digwydd ar foment allweddol i Gymru. Gyda Llywodraeth Cymru ac arweinwyr y GIG yn cyflymu’r ymdrechion i ddigideiddio, bydd y penderfyniadau a wneir nawr yn siapio seilwaith iechyd y genedl am ddegawdau i ddod. Mae’r angen i bennu’r graddfa orau ar gyfer gweithredu CCE yn fwyfwy brys oherwydd argaeledd cyllid newydd, galluoedd newydd gan werthwyr, a’r profiad cynyddol gan wledydd eraill y DU. Wrth i fyrddau iechyd baratoi i wneud buddsoddiadau strategol, mae’r uwchgynhadledd hon yn cynnig cyfle unigryw i oedi, cydgyfeirio, a phennu’r llwybr gorau ymlaen ar y cyd i Gymru.

Pynciau Allweddol a Phwyntiau Ffocws:

  • Graddfa Strategol: Modelau CCE cenedlaethol vs rhanbarthol vs lleol — beth sy’n gweithio orau i Gymru?
  • Seilwaith ac Aeddfedrwydd: Asesu aeddfedrwydd digidol ar draws byrddau iechyd Cymru.
  • Arweinyddiaeth Glinigol a Chymorth: Ymgysylltu â CCIOau, CNIOau, a thimau clinigol yn y broses ddylunio.
  • Dysgu gan Eraill: Gwybodaeth ymarferol gan yr Alban, Gogledd Iwerddon ac ICSau Lloegr.
  • Cost, Ansawdd ac Amserlenni: Deall y cyfnewidfeydd mewn dulliau gweithredu.
  • Hyblygrwydd Technolegol: Sut mae gwerthwyr yn cefnogi datrysiadau CCE graddadwy ac addasadwy.
  • Gweledigaeth ar y Cyd: Siapio blaenoriaethau buddsoddi digidol drwy bleidleisio byw a sesiynau rhyngweithiol.

Pam Mynd:

I arweinwyr digidol, clinigwyr a thimau rhaglenni yng Nghymru, mae’r uwchgynhadledd hon yn fwy na chynhadledd — mae’n gyfle prin i ddylanwadu ar gyfeiriad strategaeth iechyd ddigidol y genedl. Gyda Chymru ar groesffordd dyngedfennol, bydd y cynrychiolwyr yn clywed yn uniongyrchol gan gydweithwyr ar draws y DU sydd eisoes wedi wynebu’r un cwestiynau a llywio’r un cymhlethdodau.

Drwy gymryd rhan, byddwch yn cael mewnwelediadau ymarferol, cymryd rhan mewn trafodaethau ystyrlon, ac yn helpu i ddiffinio dyfodol Iechyd Electronig yng Nghymru.

Current Landscape and Challenges:

NHS Wales stands at a pivotal moment in its digital transformation journey. While the ambition for a connected, data-driven health and care system is clear, the path to achieving it is still emerging. Most Welsh health boards remain dependent on paper-based systems or fragmented digital tools, creating barriers to interoperability, data sharing, and clinical efficiency.

Unlike other parts of the UK, where regional and national programmes have begun to take hold, Wales must now confront a critical strategic decision: should the country adopt a single, unified Electronic Health Record (EHR) solution, or empower regional and local initiatives tailored to specific needs and contexts?

The complexity of NHS Wales’ structure—featuring distinct health boards with varying levels of digital maturity—makes this question more urgent and nuanced. The wrong approach risks wasted investment, prolonged implementation, and minimal impact on patient outcomes. The right one could unlock transformative improvements in care coordination, patient safety, and health system efficiency.

Timeliness of Event:

This summit comes at a defining moment for Wales. With the Welsh Government and NHS leaders accelerating the push for digitisation, decisions made now will shape the nation’s health infrastructure for decades to come. The urgency to determine the optimal scale for EHR implementation is intensified by the availability of new funding, emerging vendor capabilities, and the growing body of experience from other UK nations. As health boards prepare to make strategic investments, this summit offers a unique opportunity to pause, align, and collectively determine the best path forward for Wales.

Key Topics and Focus Points:

  • Strategic Scale: National vs regional vs local EHR models — what works best for Wales?
  • Infrastructure and Readiness: Assessing digital maturity across Welsh health boards.
  • Clinical Leadership and Buy-In: Engaging CCIOs, CNIOs, and clinical teams in system design.
  • Learning from Others: Real-world insights from Scotland, Northern Ireland, and English ICSs.
  • Cost, Quality and Timelines: Understanding trade-offs in implementation approaches.
  • Technology Flexibility: How vendors are supporting scalable, adaptable EHR solutions.
  • Collective Vision: Shaping digital investment priorities through live polling and interactive sessions.

Why Attend:

For Welsh digital leaders, clinicians, and programme teams, this summit is more than a conference — it's a rare opportunity to influence the national direction of digital health strategy. With Wales at a critical crossroads, delegates will hear directly from peers across the UK who have already faced the same questions and navigated similar complexities.

By participating, you'll gain practical insights, engage in meaningful dialogue, and help define the future of EHR in Wales.

NHS Case Study - Why the EPR and Shared Care Record need to co-exist: Life Outside the Hospital Walls

Karen Russell-Surtees
Digital Programme Manager
Cambridge Children’s Hospital (CCH)
Rob Nimmo
Head of Digital Transformation
Cambridgeshire and Peterborough ICS

Session Overview:

As healthcare systems move toward more integrated, person-centred models, the traditional boundaries between acute, community, mental health, and social care are rapidly dissolving. Yet digital infrastructure often remains siloed and slow to adapt.

In this session, Rob Nimmo, Head of Digital Transformation at Cambridgeshire and Peterborough ICS, argues that Electronic Patient Records (EPRs) and Shared Care Records (ShCRs) should not be seen as competing solutions, but as complementary systems that must work in tandem.

Drawing on real-world examples from across the NHS, Rob will discuss how EPRs are essential for managing care within organisations, while Shared Care Records provide the crucial connectivity needed across settings, supporting continuity of care outside hospital walls. He will challenge the myth of a single system solution and highlight the dangers of siloed digital thinking, offering practical guidance for enabling effective co-existence.

This will be an opportunity to discuss the distinct roles and strengths of EPRs and Shared Care Records and some of the key considerations around governance, consent, and trust in data sharing.

15:40
